White spot lesions are the breakdown of enamel crystals that are caused by acids, plaque accumulation, and a lack of fluoride. Local factors such as orthodontic brackets or habits such as smoking or vapor use can make things worse. Enamel decalcification is also known as the beginning of cavity formation so, in a sense, we are seeing the cavitation process when these lesions are visible. These white lesions can progress from white, to yellow, to orange/brown and eventually black lesions. We treat the milder white lesions with fluoride and good home care, but as soon as the enamel becomes soft, then treatment is required by removal of the cavity.. These can be superficial making it easy to remove through mild removal of enamel but many times run deeper, making it more difficult to restore conservatively.
